Congregation Registration Form

In order to reserve the date for your congregation, please complete this form and mail it with the required deposit prior to the deadline on May 1, 2011.

Participant Registration Form

A Participant Registration Form must be completed and returned to your Group Leader by each adult/youth participant before they travel to the Servant Event.

Participant Skills Inventory

A copy of each Participant Skills Inventory should be forwarded, as soon as possible, to Deaconess Cheadle to assist in the selection of projects that match the skills of group participants.

Medical Information Forms

Each participant should complete the Emergency Medical Information Forms and place them in a business envelope with the participant's name written on the outside.  All medical forms will be returned at the end of the servant event.
