Each summer, Diaconal OutReach Care and Services invites local social service agencies to help us identify and serve people in the community who are struggling. Together, Lutheran Church--Missouri Synod congregations from across the country stand alongside local congregations in service to our neighbors in need. The Applicant Referral Form below must be completed by each individual/family requesting assistance and signed by the case manager who makes the initial referral.
Applications should be received at least one week before the local service project begins. However, application postmarked on or before May1, 2012 will be given higher priortiy.
